Accessing Your Patient Portal & Uploading Immunizations

1. Go to the My 91´«Ã½ portal.
2. Log in using your 91´«Ã½ student credentials.
3. From your 91´«Ã½ portal homepage, locate and click on the Student Patient Portal card.
4. Once inside the Patient Portal, select Immunizations from the menu on the left-hand side of the screen.
5. Enter the dates of your vaccines.
6. Upload your immunization documentation for verification.
